Important Notice to parents:  If your child requires any medication to be given during school hours, a medication authorization form must be completed and signed by your child’s doctor and by a parent or guardian. This form must be completed annually. Do not send inhalers or other medication to school without having proper medication forms completed. School staff cannot administer medication to your child until we have completed forms. You may come to school to administer your child’s medication while waiting for  these forms to be completed.

Students entering kindergarten for the first time, must have had a physical done within one year and must have all their required immunizations.  The school must have a copy of these forms on file. NC law does not allow students to remain in school if they are not up to date on immunizations and/or don’t have a physical.  Students will be suspended from school if these requirements are not met within 30 days of school beginning or first day of enrollment (See student handbook).

Please notify me at the beginning of the school about any health concerns you have for your child and any special needs he/she may have during school hours.  If there are any changes during the year, please call me.
