ECPPS: Pasquotank Elementary School

Elementary Schools in the Elizabeth City-Pasquotank Public School District

are welcoming parents of rising kindergartners

to register their children for the 2008-2009 school year.

To register, students must be five-years-old by October 16, 2008.

Parents must bring the following items with them to register:

1. Child’s Legal Birth Certificate (Certificate must have a seal. The mother’s copy is not accepted)
2. Child’s Social Security Card
3. Child’s Immunization Record
4. Child’s Physical Form signed by the Child’s Doctor
5. Notarized Proof of Residence (such as an electric bill, phone bill, or lease agreement)

Acceptance into Kindergarten will be based on the following criteria:

1. MUST reside in the school’s attendance area
2. MUST be five-years-old on or before October 16, 2008
3. MUST have an up-to-date immunization record (shots)
4. MUST have a physical from the child’s doctor                  

These additional items are due no later than September 26, 2008.

   1.  Copy of Kindergarten Health Assessment (physical done on or after August 25, 2007)

   2. Certified Immunization Record

   3.  Student must be up-to-date on alls shots (including boosters after the age of 4)

If you fail to bring in the above documents by September 23, 2008,

your child will be sent home and may not return to school until the above requirements are met.
